sound card headphone sounds distorted

I have two in two out sound card with midi in and out Suddenly after adjusting the devices i started getting distorted sound from my headphone outlet and also speaker outlet. The speakers as it is sound verywell but when i use headphones the sound is terrible. Has my headphone preamp. gone?I have presonus audio box with me
